At Caliber Law, we understand how detrimental it can be when you have no voice when making health care decisions.
We want to help you make sure your voice is heard with specific instructions for a trusted person to speak on your behalf. Very similar to a Financial Power of Attorney, your Health Care Power of Attorney will help make your healthcare decisions when you become incapacitated or are not able to make decisions on your own, but some of these decisions are difficult and binding. Let us help you answer those difficult questions so your loved ones do not have to.
Much like a Financial Power of Attorney, the Health Care Power of Attorney helps in giving authority to a trusted person to follow through with your wishes when you no longer can. Should you ever end up in a coma or are found to be incapacitated, this document not only authorizes your Agent to access your records through HIPAA, but also allows your Agent the power to speak on your behalf without the fear of conflict. You can also choose to have two physicians decide when you are truly incapacitated prior to your Health Care Power of Attorney to take effect. This document will also allow your named Agent to execute any releases or sign off on treatment plans. We also assist in answering the more difficult questions in the added Living Will.
What if I change my mind?
Your documents can be revoked at any point in time and only requires a written and signed statement by yourself. We understand that life changes and some named Agents may move or become unavailable or you just simply want new documents. We can help you every step of the way.
